The Health Careers Summer Camp uses an online application that must be completed once it is begun. Since you may not "save as you go," you are encouraged to first work through the application offline using the PDF version of the application. If you complete your essay using a word processing program prior to completing the online application, you may simply cut and paste your essay into the online application form. There are links below to both the online application and the PDF version of the application.
To complete your application, you will need to have two teachers complete an online teacher recommendation form. There are links below to the "Request for Teacher Recommendation", which you may print off and give to the teachers from whom you are requesting a recommendation. This request form includes the URL address to direct your teachers to the online recommendation form they need to complete for your application. A complete application includes the following online forms:
- Student Information Page
- Student Personal Statement- Essay
- Two Teacher References
- One from either a Math or Science Teacher
- One from either an English or Fine Arts Teacher
